By registering on the My Edenred extranet I expressly consent to Edenred CZ s.r.o., having its registered office at Pernerova 691/42, 186 00 Praha 8 - Karlín, Company No: 24745391, incorporated in the Companies Register maintained by the Municipal Court in Prague, File No C 170804 (“Edenred CZ”), which manages card and voucher benefit programmes, processing my personal data to the extent corresponding to the reasons for my registration as:

• Employer wishing to order products to the extent: first name, surname, email address, telephone number, job title, company, name and password for logging into the extranet, IP address and cookies;

• Employee using the products to the extent: first name, surname, email address, date of birth, postal address, telephone number, type of employment, duration and status thereof, employee's identifier or personal number, information on top-ups, Ticket Restaurant® Card and/or Edenred Benefits Card serial number, name and password for logging into the extranet, IP address and cookies. I hereby confirm that Edenred CZ informed me that I am not obligated to provide my postal address, telephone number, identifier or personal number for the purposes specified below and that the disclosure of such information is optional;

• Merchant wishing to accept products to the extent: first name, surname, email address, telephone number, job title and merchant identification, bank account number, Edenred Profi Card serial number, name and password for logging into the extranet, IP address and cookies;

• Representative wishing to sign up new merchants to the extent: first name, surname, email address, telephone number, postal address, job title and employer identification, name and password for logging into the personal portal, IP address and cookies;

(the “Personal Data”) under Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016, the General Data Protection Regulation (the “Regulation”) and other generally applicable legislation. I confirm that Edenred CZ informed me that I am not obliged to provide my telephone number for the purposes specified below and that the disclosure of this data is optional, with the exception of a telephone number of the Employer's contact person. I expressly consent to Edenred CZ processing my Personal Data for the following purposes:

1. operating the client extranet,
2. managing the web-based self-service and maintaining user accounts on the client extranet,
3. importing, administering and providing employee benefits on the client extranet.

I give my consent to Personal Data processing to Edenred CZ for the above purposes and to the above extent for the duration of the validity of my registration on the client extranet. I acknowledge that Edenred CZ may store Personal Data for a period longer than the duration stated above as required by legislation.

I confirm that I understand that I have the right to request access to and rectification or erasure of Personal Data, and restriction of the processing of Personal Data, and the right to Personal Data portability. I am aware that I can exercise any of the above rights in writing to the above contact addresses of Edenred CZ or at the telephone number 234 662 300 and the email address informace-cz@edenred.com, and that I may also lodge a complaint with the Office for Personal Data Protection or another competent supervisory authority in relation to Personal Data processing. I take due note of the fact that, in order to communicate with Edenred CZ regarding the processing of Personal Data, I may also use the Edenred CZ data protection officer whose contact information is available at www.edenred.cz.

I have been informed and I agree to my Personal Data being processed by a subcontractor of Edenred CZ, the provider of the web portal, i.e. Havit, s.r.o., having its registered office at Budějovická 778/3a, 140 00 Praha 4 – Michle, Company No: 25612697, incorporated in the Companies Register maintained by the Municipal Court in Prague, File No C 54808, in respect of whom Edenred CZ guarantees that the Personal Data shall be processed to the same extent and under the same conditions as those stated above.

I am aware that I can withdraw this consent at any time in writing, by electronic communication means or by telephone and that Edenred CZ will honour the withdrawal of the Personal Data processing consent.

I confirm that by giving this consent I express a freely given, specific, informed and unambiguous agreement to the processing of my Personal data.

If I give my consent to receiving commercial messages, which is not a requirement for the provision of benefit programs, I also consent to Edenred CZ processing my personal data to the extent of my first name, surname, email address, telephone number, IP address and cookies within the meaning of Act No 480/2004 on certain information society services, as amended.
